The State Bank
The SBA has released its Form 2483, which is the formal application for a Paycheck Protection Program loan here:

Many of you have already completed The State Bank calculator, which is here.

First, complete a calculator (above) and return it to the Bank. Next (and this can be at the same time), complete and return the SBA Form 2483 Application and return it to the Bank.
When the Bank receives BOTH a completed calculator AND a completed application, I will forward you a DropBox or other secured method so that, when we know exactly what is needed, you can upload it. Most likely you will need: Organizational documents (operating agreement, partnership agreement, articles of incorporation or organization, by-laws, etc.
– Copy of the driver’s license of the principal
– Detailed information around past payroll (the top half of the calculator)
– Detailed information about the projected payroll and other eligible expenses
Please complete the calculator AND the application, and let’s get this done!
